Patients can also order medication and prescription items online using the Patient Access System. To use this system you will need to register by completing the attached application form. You will need to return your completed application form to the Practice together with 2 x forms of identification, one must be photographic ID (Passport or Driving License) and one must have your current address recorded (Utility Bill or Bank Statement).Online Access Registration Form
