How to Register

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

The catchment area includes the following:

  • Sutton Farm/Springfield
  • Monkmoor
  • Coleham
  • Belle Vue
  • Reabrook
  • Copthorne
  • Bicton Heath
  • Radbrook
  • Bayston Hill
  • Atcham
  • Cross Houses
  • Upton Magna
  • Sundorne

The list is not exhaustive.  If you are unsure whether you fall within our catchment area or not, please contact the practice and a Receptionist will be able to advise you.

You can register at the practice by completing the downloadable New Patient Registration Questionnaire below and returning it to the practice. Our address can be found on the homepage of this website. You can also email the completed registration form back to us at marden.marden@nhs.net.  Registration forms are also available from Reception at the Practice.

You are a patient of the practice by successful receipt of your completed registration form and acceptance of that registration form by the practice.

We recommend that new patients undertake a Health Check with the Health Care Assistant and to assist us in this process we will ask you to complete a new patients questionnaire, which includes details of your ethnic grouping and smoking status.

TEMPORARY RESIDENT

You can register temporarily with a GP surgery while away from home for work, study or on holiday. You’ll remain registered with your permanent GP surgery.

Details of any treatment you have will be passed  to your permanent GP surgery. They will add the information to your medical records.
